A partir de este momento, todo participante con licencia escolar deber\u00e1 llamar al nuevo tel\u00e9fono 911.59.98.54 (con Asistencia Central de SOMPO) para recibir la asistencia sanitaria correspondiente.<\/p>\n<p><strong>Datos relevantes a conocer cuando se realiza la llamada<\/strong><\/p>\n<p>Nombre del tomador: Gipuzkoako Foru Aldundia<br \/>\nN\u00famero de p\u00f3liza (diferente en funci\u00f3n de la actividad):<br \/>\n&#8211; Grupos de Deporte Escolar: LAH3120065<br \/>\n&#8211; Grupos de la Campa\u00f1a de verano: LAH3119077<\/p>\n<p><strong>Otros datos<\/strong><\/p>\n<p>Se solicitar\u00e1 el DNI del asegurado, tel\u00e9fono de contacto y correo electr\u00f3nico, domicilio y una declaraci\u00f3n sobre lo ocurrido.<\/p>\n<p>De momento no se ha facilitado informaci\u00f3n para tramitar ning\u00fan \u00abparte\u00bb.
